A Maine advance health care directive form allows you (the “principal”) to select someone (the “agent”) to make decisions about your health care treatment if you become incapacitated. In Maine, this document is commonly known as a power of attorney for health care.

Statute: Title 18-C, Article 5, Part 8: Uniform Health Care Decisions Act.
Signing Requirements: Two (2) Witnesses (§5-803-(2)).
Revocation: It can be revoked in total or partly, in writing or by personally informing the health care provider. If an agent is your spouse and your marriage ends in divorce, annulment, dissolution, or legal separation, the power of attorney is revoked unless otherwise specified in the form (§5-804.)
