This document is applicable to a single vehicle owned and controlled by the principal.
Recognized by the Idaho Transportation Department (ITD), the form requires the vehicle details, information about the attorney-in-fact (name and address) and the grantor (name, driver’s license number/SSN/EIN, address, and phone number), and their signatures.
