A Wisconsin medical power of attorney form gives someone you trust the right to make healthcare decisions for you. The person you appoint is called an agent and makes decisions for you when a doctor determines you can’t do so yourself. While you’re still healthy, you maintain authority over your medical care. In Wisconsin, this form is also referred to as a power of attorney for health care.

- Signing Requirements: Two witnesses (§ 155.10(1)(c)).
- The witnesses must not be the principal’s domestic partner or related to the principal by adoption, blood, or marriage. Furthermore, they must not know that they’re entitled to the principal’s estate.
- Revocation: The principal can create a new POA to nullify the existing one, verbally revoke it, or destroy the written record (§ 155.40(1)).
