An Oregon tax power of attorney form, Form 150-800-005, or “Tax Information Authorization and Power of Attorney for Representation,” is a legal document that enables a taxpayer to appoint a representative, like a family member, attorney, or CPA, to handle their state tax matters with the Oregon Department of Revenue. This form grants the representative access to tax documents and authority to file taxes, with the taxpayer able to specify the scope of this power and the tax years involved.
While it’s recommended that the representative be a qualified professional, it’s not a legal requirement. The taxpayer’s execution of this POA automatically nullifies any previously filed powers of attorney for the same tax matters and years unless the taxpayer otherwise indicates with their initials.
