A Utah tax power of attorney form, or Form TC-737, is a legal document that lets a taxpayer appoint a tax representative to exercise authority over tax-related matters with the Utah State Tax Commission. The taxpayer can name up to two representatives on the original form, but they can attach additional papers as necessary to name more than two agents.
This Power of Attorney and Declaration of Representative form allows the representative to receive and review confidential tax information, prepare various tax-related documents, and appear in proceedings before the Commission for the tax matters and tax periods that the taxpayer indicates.
