A Louisiana medical power attorney lets you give another person, called your agent, power over your health care decisions should you become incapacitated. Your agent must carry out your health care wishes according to the instructions you leave in the form. This document is also known as a power of attorney for health care form.

Signing Requirements: Two (2) Witnesses or a Notary Public (RS § 40:1151.2).
Revocation: You can revoke at any time, even after incapacitation (RS §40:1151.3).
