A New York Medical Power of Attorney, or health care proxy, is a document that lets you assign a trusted person as your health care agent. Your agent is responsible for making medical decisions if your doctor decides you’re too sick to communicate responsibly.

Definition Article 29-C – 2980(8): “Health care proxy” means a document delegating the authority to make health care decisions, executed in accordance with the requirements of this article.
Signing Requirements: Two (2) Witnesses (PBH § 2981(2)).
Revocation: You can revoke your health care proxy unless a court decides you’re too sick to make decisions for yourself. To revoke, you can notify your agent or doctor that you want to revoke your proxy, create a new form with different terms, or sign a revocation form and share a copy with your doctor or agent (PBH § 2985).
