An Oregon medical power of attorney lets you assign an agent to make medical decisions if you become unable to make them. The agent will be able to make medical decisions you would make yourself if you were competent, such as proceeding with or refraining from treatment.

- Definition: An “advance directive” includes a form appointing a health care representative and instructions to the representative, along with any supplementary document or writing attached by the principal (ORS 127.505(2)).
- Signing Requirements: Two witnesses or a notary public (ORS §127.515).
- Revocation: Make a new one with different conditions, notify your doctor, or create a written revocation form, and if your spouse is your agent, getting a divorce or annulling your marriage will suspend it (ORS §127.515).
