A Virginia Medical Power of Attorney form is a document that lets you choose someone to be in charge of critical healthcare decisions if you become incapacitated. The person you appoint, called your agent, can only start making decisions on your behalf if you can’t communicate your wishes.

Revocation § 54.1-2985: You can do so in three ways: sign and date a Revocation of Power of Attorney document, verbally express your decision, or destroy the original document. Whichever method you choose, it is essential to communicate the revocation to your agent and healthcare professionals.
